Please have your insurance information prepared prior to scheduling. We cannot bill your insurance if we don’t have the correct information! Please bring the cards with you to your appointment, we will take a copy at check-in. 

Health Insurance

We accept most major medical insurance.

Please provide your current policy information when making appointments. Some policies offer coverage for routine eye exams.

If you are having a medical problem, are being seen for a diabetic eye exam or followed for an eye disease like glaucoma or macular degeneration, your medical insurance will be billed.

Vision Insurance

Vision Service Plan (VSP): Your benefit information can be found at vsp.com

In order to verify your coverage, we will require the policy holders full name, date of birth and social security number.


Eyemed/Blue View Vision: Your benefit information can be found at eyemed.com

This provider has several different policies. We are not in-network with all of them. In-network providers will be listed in your individual policy information.

These plans cover wellness eye exams only. Most policies offer coverage for glasses or contact lenses. This may be a funded benefit or a discount based on your individual policy.

No Insurance?

We are happy to see patients with no insurance coverage. Payment is due at time of service and we accept all major credit and debit cards, cash and Care Credit.

We encourage patients to check their benefits prior to their appointment. If you have any questions regarding your policy or coverage, you should contact your insurance provider directly.

Our insurance specialist will confirm that your policy is open and active and what co-pays may be due at time of service. This is not a guarantee of payment by your insurance company. If you receive a bill and have questions, please call the office promptly.
