Please bring all eye wear including sunglasses to your appointment.
Please bring a current list of all medications to your appointment. Many medications can affect your eyes and vision.
Please make sure to bring all insurance cards that may be used for your eye exam. Some insurance plans have routine vision coverage, others may cover part of a medically necessary eye health examination.
Please make sure you obtain a referral from your PCP, if necessary, prior to your visIt.
